Job duties specific to this position:

CLINICAL RESPONSIBILITIES

  • Assumes a primary role and responsibility and accountability for the provision of safe and effective drug therapy and other pharmaceutical care services intended to achieve outcomes related to the cure, diagnosis or prevention of disease, elimination or reduction of patient’s symptoms, or arresting or slowing of a disease process, and improve a patient’s quality of life in collaboration with a multi-disciplinary team.
  • Evaluates, resolves, and documents, according to established policies and procedures, potential drug therapy problems identified through any and all available sources, including any pharmacy information system.
  • Develops individual dosing regimens based on pharmacokinetic principles, medication history, age and disease state.  Monitors  and assesses the outcome of drug therapies including physical assessment and interpretation of laboratory and other diagnostic parameters.
  • Provides advanced pharmacology and therapeutic consultation on formal and informal basis concerning medication management to other healthcare staff.
  • Develops, plans, coordinates, and documents clinical pharmacy services in respective area of practice.
  • Monitors patients for appropriate laboratory monitoring parameters, drug-drug interactions and co-morbidities, duplication of therapy, medication side effects, and appropriate dosing regimens, absence of essential medication, drug allergies, and occurrence of adverse events in order to implement and/or recommend corrective actions.
  • Confers with medical personnel concerning care and treatment of patient’s related clinical diagnosis, drug combinations and dosage forms, and other factors that might influence the course of treatment and the activity of the drugs, including drug identification, suggests changes in drug therapy and/or use, as appropriate.
  • Interviews patients to obtain information regarding drug use, drug allergies and sensitivities, documents the information in the patient’s medical record or pharmacy information system, advises patient on significant precautions, proper drug therapy and administration, the use of related devices, and the coordination of drug therapy with diet, according to established policies and procedures.
  • Actively seeks information to understand health care professional and patient circumstances, problems, expectations, and needs. Considers how actions or plans will affect members; responds quickly to meet patient needs and resolves problems.
  • Provides input for planning and implementing pharmacotherapy related issues characterized by education, program management, and clinical practice such as assisting with the development of pharmacy educational programs.
  • Participates in developing criteria for safe and effective medication use as well as monitor the compliance with established criteria and other quality improvement programs such as the Medication Use Committee of the Pharmacy and Therapeutics Committee (P & T).
  • Evaluates clinical trending reports and other quality assurance activities as indicated to ensure appropriate and safe drug utilization; organizes, designs, and conducts drug utilization evaluations.
  • Oversees dispensing and distribution of pharmacy related products according to legal requirements, established policies and procedures and acceptable professional practices.
  • Documents responsibility by signing or initialing prescriptions, batch sheets or other forms according to established policies and procedures for controlled substances, non controlled substances, compounds, patient specific IV’s, chemotherapy and TPN solutions.
  • Maintains competency in the preparation of patient specific IV’s, chemotherapy, and TPN solutions.
  • Accepts the responsibility for resolving operational problems, in the absence of, or as assigned by, supervision, such as resolving professional problems related to the interpretation of prescriptions, drug furnishing irregularities, service complaints and technical problems such as equipment needs and drug procurement, etc, according to established policies and procedures.
  • Provides direction to pharmacy technicians, students and other personnel assuring all operational and distribution tasks are completed.  Makes suggestions and answers questions.
  • Check equipment used to prepare and dispense medications for proper functioning, report deficiencies to appropriate personnel
  • Maintains confidentiality of patient records and medical center information at all times.
  • Applies knowledge of the standards related to distribution and control of scheduled and non-scheduled drugs.  This includes, but is not limited to, the standards established by DEA, DPH, FDA, and TJC.
  • Maintains records of drugs and controlled substances as required by federal and state law.
  • Maintains records pertinent to the purchase and control of narcotics and alcohol, following established procedures.
