The goal of the Case Manager is to facilitate the delivery of optimal healthcare while promoting fiscal responsibility. Participates in the coordination and access to care for members with high cost/high risk conditions. The Case Manager will address member needs after the completion of a health assessment and develop appropriate member focused interventions. In addition, the CM evaluates available resources, plans, and implements necessary services in order to monitor outcomes of identified problems. As an integral member of the Care Team, the CM will collaborate with other members of the health care team to coordinate an integrated approach to member management. This role may be hospital-based, community-based, long-term facility-based or telephonic.
